11/25/2023 EnchantaVaughn Approved Behavior: Mother reported that client struggles with getting along with peers in the home and community setting. She reports isolating herself at least 2-3 times daily with a moderation lasting 3-4 hours. Behavior: Client self-reported to QMHP on how she strugglues with communicating with others and when on a daily basis and will not talk to others when she does not want to for 2-3 hours in duration. Intervention: QMHP went through program overview with family to reiterate key points and program objectives that would pertain to their specific situation and helping to build a new therapeutic relationship with the family and client without making them feel overwhelmed with questions or being put on the spot. QMHP encouraged client to continue displaying positive behaviors. QMHP supported client involvement in her treatment. QMHP praised client for her cooperation throughout the session. Intervention: QMHP will explore positive self-talk, use instruction modeling, and increase social communication skills with positive reinforcement role play activities. QMHP processed with client about what she learned from activity after the review. QMHP will educate client on behavior modification to help client identify what behavior is appropriate and acceptable. QMHP will utilize behavior modification system to aid the client with adjusting to her current lifestyle. Response: Client expressed positive thoughts about herself. QMHP. Client responded in a positive manner acknowledging the positive reinforcement that the QMHP provided. Client stated she didn't need help with anything. Client communicated with positive gestures and exhibiting good eye contact. Client verbalized positive feelings about achieving her goals. Client communicated that she wants to work independently, so that she could gain clarity of her personal strengths and attributes. Response: Client displayed good participation and was responsive to all instructions, interacting well with mom and QMHP. Client responded well to feedback. Client understands her treatment plan is important with helping her build better work ethic, self-esteem, and mental functioning. Client acknowledges her family support and agrees to be more thankful. Client implemented the positive reinforcement provided by her counselor and was able to stay motivated throughout the session.
